Pt Family medicine policies

Insurance Information

PT Family Medicine, PC is credentialed with multiple, but not all, insurance companies. While PT Family Medicine, PC will do its best to communicate its credentialed status (e.g. par, non-par, in-network, out-of-network) prior to services being rendered, the ultimate responsibility of knowing this status is that of the patient's and/or the patient's responsible party. PT Family Medicine, PC advises patients and/or their responsible party to contact the patient's insurer to confirm PT Family Medicine, PC's network status and the details of their coverage (e.g.co-payments, deductibles, co-insurance, lab contracts). As a service to the patient's we care for, PT Family Medicine, PC will bill patient's insurers, if applicable, for the medical services rendered.Payments from this process will be paid directly to PT Family Medicine, PC. The patient and/or the patient's responsible party are responsible for providing accurate insurance information, and any fees related to incorrect or invalid insurance information being provided.

Outside Services

Despite specimens being obtained while receiving care at PT Family Medicine, PC, certain tests are performed by separate, outside companies. In these situations, patients and/or their responsible party are responsible for working directly with these companies to remedy any balances due from the services they rendered.

Other Fees

The following scenarios will result in the corresponding additional fees being applied, and will be the responsibility to pay at time of service(s) rendered:

Returned Check: $25 per check

Late Cancellation/Missed Appointment: Fees as per PT Family Medicine, PC's Late Arrival, Appointment Cancellation, and No Show Policy

Form Completion Fee:

  • Simple (One Page) - $10; Handicap Parking Permit, Letter
  • Detailed (Two Pages) - $20; FMLA Paperwork, Short Term Disability
  • Complex (3 or More Pages) - $25; Permanent Disability, Facility Admitting

Medical Records: Charged according to the Pennsylvania’s Department of Health guidelines and fees for providers and medical facilities.

Financial Responsibility

By electing to receive care at PT Family Medicine, PC, the patient and/or the patient's responsible party assumes financial responsibility for medical services not covered by their insurer(s), as well as for any services deemed as cosmetic. Some payments for medical services are due at the time services are rendered (e.g. copayments), while others are due at

the time they are determined/statement regarding such are provided. Payment for cosmetic service(s) is (are) due at the time services are rendered. If a patient's account balance remains unpaid and is transitioned to a collection agency, the patient and/or the patient's responsible party assumes all costs of collection. Patient's with unpaid balances transitioned to a collection agency may not be permitted to schedule follow-up appointments, until their balance is remedied.

Thank you for trusting your medical care to PT Family Medicine, PC. When you schedule an appointment with our office, we diligently work to schedule an appropriate amount of time to best accommodate your needs, and ensure enough time to provide you with the highest quality of care. Should you need to cancel or reschedule an appointment, please contact our office as soon as possible, and no later than 24-hours prior to your scheduled appointment. Please see our updated Late Arrival, Appointment Cancellation, and No Show Policy. Effective immediately:

  • Patients are asked to arrive at their scheduled appointments before their scheduled appointment time. New patients are to arrive 30 minutes prior to their scheduled appointments. Established patients are to arrive 15 minutes prior to their scheduled appointments.
  • If a patient arrives late for their appointment, the patient will be asked to reschedule. This is to ensure that the patients who do arrive on time are seen in a timely manner.
  • Any patient who fails to show or cancels/reschedules an appointment and has not contacted our office with at least 24-hours notice will be considered a No Show, and will be charged according to our sliding fee for offenses.
    1. New Patients will be given a courtesy for their first offense; subsequent failures to show will result in the inability to reschedule a patient.
    2. First offense for Family Medicine will result in a $25 fee; First offense for Sports Medicine will result in a $35 fee.
    3. Second offense for Family Medicine will result in a $35 fee; Second offense for Sports Medicine will result in a $45 fee.
    4. Third offense for Family Medicine will result in a $45 fee; Third offense for Sports Medicine will result in a $55 fee.

      After three missed appointments, the practice may at its discretion choose to discontinue your care.

  • All fees accrued must be paid before a new appointment can be rescheduled.

We understand there may be times when unforeseen circumstance(s) arise, and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances, please contact our Office Manager. Fees may be waived at the discretion of the Office Manager and/or the Provider, and the circumstances at hand. Should you call after our office’s regular business hours, or over the weekend, you may leave a message with our answering service. Messages can also be sent via our patient portal.

To ensure the safety of our patients here at PT Family Medicine, PC, the following policies are now in effect:

1. Symptom Reporting
  • If you are experiencing any of the following symptoms, you must call our officeprior to your appointment to check in, and await further instructions from our staff:
    • Fever or chills
    • Cough
    • Shortness of breath or difficulty breathing
    • Fatigue
    • Muscle or body aches
    • Headache
    • New loss of taste or smell
    • Sore throat
    • Congestion or runny nose
    • Nausea, vomiting, and/or diarrhea
2. Contact and Testing
  • Please inform our office if you have:
    • Been in close contact with anyone who has tested positive for COVID-19 or influenza
    • Tested positive for COVID-19 or influenza in the past 14 days
    • Are currently waiting for the results of a COVID-19 or influenza test
    • Traveled internationally or to a high-risk area in the past 14 days
3. Mandatory Symptom Checklist
  • All patients must complete a mandatory symptom checklist upon arrival to their appointment.
4. Office Procedure
  • A staff member may ask you to wear a face mask and/or return to your car to be tested for COVID-19/Flu depending on your responses to our mandatory symptom checklist and/or an increase in illness in the community. This is being asked of your own safety, and the safety of others you come in contact with while in our office.
  • Any patient refusing to follow our office protocol will be asked to leave the premises, and their appointment will be cancelled and classified as a No-Show, resulting in the patient being responsible for paying a no-show fee before they are permitted to schedule further appointments with either Family Medicine or Sports Medicine.
5. Falsification or Failure to Report Symptoms
  •  Patients who falsify the reporting of symptoms, or fail to report symptoms at the time of check-in, will be in violation of our office policies. This is crucial to maintaining a safe clinic for all patients, and a secure work environment for our staff.
  • Any falsification or failure to report symptoms will result in dismissal from the practice, and may lead to legal action.

We take these measures incredibly seriously to provide a safe and secure environment for everyone. Thank you for your cooperation and understanding.

With the upcoming winter weather, PT Family Medicine, PC would like everyone to be aware of the inclement weather policies we have in place:

  • If there is a snow emergency, the office will be closed, and all cancellation fees will be waived.
  • If you are going to be late due to the snow, please reach out to us so we can adjust or reschedule your appointment, and no fees will be charged.
  • If we have to close the office early due to snow, we will communicate with everyone as soon as possible, make a post on social media, and we will do our best to get you rescheduled as soon as we can.
